Now, holding a vessel filled with the nectar of medicine, adorned with the marks and signs, and seated in the vajra posture, visualize the Medicine King surrounded by countless retinues like a gathering of clouds, including the Buddhist retinue such as the Three Family Protectors, the retinue of sages such as the son of Changshi, the retinue of gods such as Brahma and Indra, the retinue of outsiders such as Mahadeva, the medicine goddesses of the five elements, and the retinue of vidyadharas. From the crown of the Medicine King, radiate the wisdom of the dharmadhatu of Vairochana; from the heart, radiate the wisdom of the dharmadhatu of Vajrasattva; from the navel, radiate the wisdom of the dharmadhatu of Ratnasambhava; from the secret place, radiate the wisdom of the dharmadhatu of Amoghasiddhi, these four wisdoms all have the form of the four Buddha families. From the throat, emanate the mind-born form of Amitabha. The mind-born one acts as the great questioner. The wisdom of the dharmadhatu of Vajrasattva transmits the root mind stream; the wisdom of the dharmadhatu of Vairochana transmits the spoken body stream; the wisdom of the dharmadhatu of Ratnasambhava transmits the instruction quality stream; the wisdom of the dharmadhatu of Amoghasiddhi transmits the subsequent action stream, giving empowerment, permission, and entrustment in sequence. These four also descend from the sky like rain in the form of blue, white, yellow, red, and green letters, dissolving into your crown, heart, navel, and secret place, visualize that the meaning of these letters clearly arises in your mind stream. After that, all these retinues also receive the empowerment of all the medical texts of their respective systems. These letters transform into white and red light, dissolving into oneself. Finally, all the other retinues dissolve into the mind-born one. The mind-born one also transforms into red light and dissolves into your throat, therefore you completely understand the meaning of the four tantras, and generate the pride of being able to speak these words without hindrance. Then, the four wisdoms dissolve into the Medicine King, the Medicine King transforms into light from his own crown and dissolves into oneself, visualize that it is indivisible like water dissolving into water. Placing the scripture on the head, the five defilements of beings are born from the cause of the five poisons, afflicted by the five diseases of wind, bile, phlegm, and combination, with the five wisdoms and five medicinal preparations, eliminate the five-five diseases of cause and effect, the wisdom of the dharmadhatu.

Arising from the five wisdoms, the four great tantras with their five concluding sections, the eighteen branches with their accessories of healing, by the empowerment that gathers all the teachings and instructions, may I obtain the power to understand the words and meanings as they are. Seventh, Medicine: Holding the skull cup of medicine, by blessing the skull cup, mirror, and medicine together as the nature of nectar, placing it on the crown of your heads, may you realize the appearances and existence as the nature of nectar medicine, and may you obtain the power to eliminate the suffering of diseases gathered by cause and effect. The skull cup is the outer vessel, the realm of the world, the basis for generating earth, water, fire, wind, and all medicines. Filled with nectar is the inner essence, sentient beings, flesh, blood, warmth, and breath are all transformed into medicine. The mirror is a symbol of the appearance of coolness and warmth separately. Diseases of heat and cold generated by afflictions of attachment and aversion, eliminate them with the medicine of skillful means, camphor, and Rhododendron. Ignorance, attachment, and aversion mixed together, heat and cold fight. Generation and completion, union, Terminalia chebula balances the elements. The six realms, ignorance, five aggregates, six sense bases diseases, destroy them with the six perfections and the six excellences. The realm of appearances and existence, samsara and nirvana, the great nectar, by bestowing this supreme empowerment of all medicines, may you obtain the power to eliminate all diseases of cause and effect.
