啊，修持一切护法，将生圆次第的禅定推向极致，赋予无碍成就事业的教诲。将根本心髓之续部的教诲交付，平息众生的嗔恨之因果疾病吧！将讲解身之续部的教诲交付，平息众生的愚痴之因果疾病吧！将口诀功德之续部的教诲交付，平息众生的傲慢之因果疾病吧！将后续事业之续部的教诲交付，平息众生的嫉妒之因果疾病吧！将结尾意义完全交付的教诲交付，平息众生的贪欲之因果疾病吧！将内外秘密的誓言教诲交付，引导一切具缘者走向大乐之道！将修持事业和誓言物交付，恒常竖立修持的胜幢！将药物和医疗器械的一切交付，尽力利益一切病人吧！我将此教诲交付于你，愿一切经论口诀事业都能圆满成功，教规不衰，佛法兴盛广大，一切众生皆得安乐！如是说后，给予甘露，进行开光，伴随乐器，宣说吉祥。之后，秘密隐藏共同誓言的教诲：作为身的誓言，为了使身稳固，持有具加持力的佛像或身像不离身；持有头发替代品卡杖嘎和金刚杵。为了使语稳固，持有生圆次第的口诀，《四续》及其所有部分，将所有词句毫无遗漏地铭记于心；中等者每月念诵一遍，恭敬思维其意义；下等者也每年念诵三遍，思维其意义。持有铃、铙钹、骨号、笛子、颅骨鼓。作为意的誓言，不离生圆次第的禅定而修持。持有具加持力的甘露丸，不损坏金色的五角星，以及用金子书写的心髓。作为功德的誓言，为具器者讲解并传播生圆次第的口诀和《四续》及其所有部分。作为事业的誓言，对于一切病人，无论病情轻重，都无有亲疏远近，尽力利益。备齐一切药物和医疗器械。祈请一切护法。

Ah, practice and accomplish all the protectors, entrust the teachings to bring the samadhi of generation and completion to the ultimate end, and to accomplish activities without obstruction. Entrust the teachings of the root mind lineage, pacify the causes, effects, and diseases of hatred of sentient beings! Entrust the teachings of the explanatory body lineage, pacify the causes, effects, and diseases of ignorance of sentient beings! Entrust the teachings of the instruction quality lineage, pacify the causes, effects, and diseases of pride of sentient beings! Entrust the teachings of the subsequent activity lineage, pacify the causes, effects, and diseases of jealousy of sentient beings! Entrust the teachings of the concluding meaning completely, pacify the causes, effects, and diseases of desire of sentient beings! Entrust the inner and outer secret samaya teachings, guide all fortunate ones to the path of great bliss! Entrust the practice, accomplishment, activities, and samaya substances, firmly establish the victory banner of constant practice! Entrust all medicines and medical instruments, benefit the sick to the best of your ability! By entrusting this teaching of mine to you, may all scriptures, instructions, and activities be successfully completed, may the tradition not decline, may the doctrine flourish and expand, and may all sentient beings be established in happiness! After saying this, give nectar, consecrate, and proclaim auspiciousness with music. Then, the general samaya advice is secretly hidden: As the samaya of the body, to make the body stable, hold a blessed Thangka or body image inseparable. Hold a hair substitute khatvanga and vajra. To make speech stable, hold the instructions of generation and completion, the Four Tantras and all their parts, and memorize all the words without omission. The intermediate one should recite it once a month, respectfully contemplate its meaning. The lowest one should also recite it three times a year and contemplate its meaning. Hold a bell, cymbals, bone trumpet, flute, and skull drum. As the samaya of the mind, practice the samadhi of generation and completion without separation. Hold the blessed nectar pill, the undamaged golden five-pointed star, and the essence written in gold. As the samaya of quality, explain and propagate the instructions of generation and completion and the Four Tantras and all their parts to worthy vessels. As the samaya of activity, for all patients, regardless of the severity of the illness, without near or far, benefit to the best of your ability. Gather all medicines and medical instruments. Pray to all the Dharma protectors.

It should be offered as a constant offering. These vows, may oneself, Dharma friends, and the three Gurus not be ashamed, and may one possess mindfulness and awareness. It is necessary to protect them at all times, with great importance and strict admonition, by entrusting them to the Dharma protectors. It was taught by the Medicine Guru Yuthokpa himself, so do not abandon it, but protect it properly. Thinking in this way, repeat after this. "How the Lord..." etc., recite three times. Offer the extensive thanksgiving mandala, offer body and possessions, and dedicate the merit to enlightenment. Perform as in the common ritual, and then the disciples depart. Third, the order of the subsequent activities is: according to the ritual manual, invite the assembly blessing and the assembly field. Add the offering, confession, and the fulfillment prayers of the Three Roots, and the fulfillment of the Dharma protectors' power. Perform subjugation and suppression, and then offer the torma. Enjoy the assembly, and dedicate the leftovers. Perform breaking vows, stabilizing, dancing, receiving siddhis, and perform offerings, praises, and confess transgressions. For thangkas etc., perform the stable abiding, and in the case of tsok, generate the front and invite to descend. Generate oneself, dissolve and absorb, dedicate aspirations, and perform auspiciousness completely. For actually accomplishing benefiting others, in medicine: there is little in the world with greater qualities than this. Relying on it, one can easily attain enlightenment. This essence of the method of accomplishment is the most excellent of all. Its ripening cause, including root and branches. By the virtue of compiling and arranging it into a readable form, may all sentient beings pacify the suffering of cause and effect. May the unsurpassed Medicine King quickly be accomplished! The extensive empowerment of Yuthok's Heart Essence, together with the appendix and oral instructions, are compiled together, and made clear by the Guru's practice and oral transmission.
