Unstained and flawless deity of the Padma family, Immortal Lord of Life Amitayus (Tibetan: འོད་དཔག་མེད།), Lord of Compassion Avalokiteśvara (Tibetan: སྤྱན་རས་གཟིགས་དབང་སྐུ), Lotus-Born Vajra (Tibetan: མཚོ་སྐྱེས་རྡོ་རྗེ།) Vidyadhara, deity of life, may the wisdom of the Sugata's compassion be auspicious! Oh, thereby purifying the accumulated obscurations of equality, receiving the fourth empowerment, endowed with the opportunity to accomplish the fruit of the Dharmakaya, obtaining the spontaneously accomplished empowerment. Now, in order to fully request the life empowerment, please repeat this supplication. Lotus-Born Vajra (Tibetan: མཚོ་སྐྱེས་རྡོ་རྗེ།) father and mother, and the Vidyadharas who have power over life, by obtaining the immortal Vajra empowerment, please grant the complete life empowerment. Once or three times. This empowerment torma, located at the center of the thousand-petaled, seven-tiered lotus tree, within the vast expanse of the wisdom kapala. The torma is like a wish-fulfilling jewel, amidst the diverse radiating clouds of offerings, at the summit of the lotus tree, within the vast and expansive palace, Guru Maha Lotus-Born (Tibetan: མཚོ་སྐྱེས་རྡོ་རྗེ།) inseparable in union with the consort Vajravarahi (Tibetan: ཕག་མོ།), to the right is Mandarava (Tibetan: མནྡཱ་ར་བ།), to the left is Yeshe Tsogyal (Tibetan: ཡེ་ཤེས་མཚོ་རྒྱལ།), on the four petals are the four classes of dakini parents, below on the eight lotus petals are the Guru's eight names parents, all with red bodies, holding the vase of life. On the lotus petals outside the bhanda (Tibetan: བྷནྡྷ), in order, are the lineage masters of the Vidyadharas who have power over life, the deities of the sutra and tantra of Buddhas and Bodhisattvas, the assembly of heroes, dakinis, dharma protectors, wealth deities, and all including the eight classes of gods and demons, the six realms of sentient beings, visualize all transformed into the body of the yidam deity, holding the vase of life. Then place this torma on the crown of the head.
