Consider the harmer of harm as a debt. Now, there is nothing in the mind, thinking, 'Let whatever comes, come,' without fear or restraint, without pride or thought, naturally relaxing and abiding, letting awareness merge into the nature of reality. The illness will then be cured. The method of trampling with the feet in action is: No matter which deity or demon causes harm, one should engage in mad conduct in the fierce places where they dwell. This has already been explained in detail before. Diseases arise from the cause of karma and the condition of afflictions. They are expelled through wind and illusory wheels, methods, and focus. This speaks of examining the diseases of the elements and the two methods of expelling them. Examining diseases is: Even four hundred and four types of diseases are born from the four elements as their cause. The condition is unresolved conflict. The essence is condensed into two: hot and cold. Thus, all diseases are born from the four elements as their cause: earth causes phlegm, water causes bile, fire causes wind. The condition mostly arises from unresolved conflict. The result is that all diseases are condensed into two: hot and cold. The first is: The disease of earth, the body is heavy, awareness is unclear, there is excessive sleep, bad breath, and discomfort in appetite. The disease of water is: The body trembles with cold, there is cold sweat, belching occurs, food is not digested, it changes unpredictably, there is diarrhea and vomiting. The disease of fire is: From the forehead, there is hot trembling, irritability, impatience, fever, thirst, a desire to drink water, and confusion. The disease of wind is: Awareness is distracted, there is itching, dizziness, incoherent speech, a desire to walk, dry mouth, and noises and buzzing in the ears. The methods of expelling diseases are three: the preliminary is prayer, the main part is meditating on peace and non-objectification, and the conclusion is to leave it as one pleases. First, arrange whatever offerings, worship, and torma are available. Invite all the lineage lamas, such as Ama Jomo, and pray that one may attain enlightenment through this illness, or in the intermediate way, pray that this illness may be turned into a path and purify obscurations, or in the lowest way, pray that this illness may be quickly cured. In scattered instructions, it says: When afflicted by diseases arising from the elements, without distraction, meditate on the guru. Pray earnestly and close the lower door. And so on. The main practice of focusing has three parts: blending the outer and inner elements into one, the four elements

Equilibrium and applying disease to disease. The first is for external diseases, no matter which of the four elements' disease occurs, the body stands up with the disease, the body stands straight, the two feet are close together, and the two hands are joined in prayer at the crown of the head. Breathe gently, hold your breath, and focus your attention. Merge the four external elements into the four elements of your own body: merge earth into flesh and bones, water into blood and serum, fire into warmth, and wind into breath. Merge into where it is needed, attract and mix the two internal and external elements without distinction, and in that state, merge the mind into the sky and maintain emptiness. Then put the painful place down, completely abandon the body without self, release the mind without support, and walk in the sky when walking. Staying, eating, and doing anything are done in the sky. By doing this to all four elements, the external disease will be cured. For internal diseases, from within the body, merge the four elements of your own body into the four external elements, such as merging flesh and bones into earth, and so on, and keep the mind without attachment. Let the clinging disappear like a mist, abandon the body, and meditate, lie down, and sit. In that state, naturally fall asleep, and when you wake up, the disease will be cured. Balancing the four elements means that for diseases of the four elements, it is caused by imbalance, and in order to balance, you must know how to increase and decrease. For hot diseases, the power of fire is great and the power of water is small, so meditate on water on the ground, increasing cold and decreasing heat. Similarly, meditate on fire for cold, earth for wind, and wind for earth, so that it is balanced. Apply hatred to the disease of desire, and desire to the disease of hatred, gather the scattered, and scatter the gathered. Applying disease to disease means that when suffering from fever, relying on food and behavior that are harmful to fever will lead to liberation. Fever is a method of pacifying with fire. For example, if you put a fire mirror, or a chimney, or iron with rust in the fire, fire will not occur later. For cold diseases, relying on cold-important food and behavior will pacify and heal, cold is a method of destroying in water. For example, if you put ice in water, it will melt. Similarly, for wind, eating beans will cause the wind to leave the body.
