Says: To regard the Dharma speaker as a doctor is like this: For example, when suffering from severe illnesses such as wind and bile disorders, one seeks a skilled doctor. When one meets such a doctor, one is overjoyed, listens to what the doctor says, and respectfully attends to the doctor. Similarly, one should seek a Dharma-teaching spiritual friend in the same way, and upon finding one, not regard them as a burden but as an adornment, carrying out their instructions and respectfully attending to them. As it says in "Gathering Precious Qualities": Therefore, those who seek supreme enlightenment should, with strong intention, have their pride thoroughly destroyed by the wise. Just as a gathering of patients relies on a doctor for healing, one should rely on a spiritual friend without distraction. This is why it is said. To cultivate the perception of teachings as medicine is like a patient greatly cherishing the medicine prescribed by a doctor. One should see the instructions and subsequent teachings of the Dharma speaker as very important, cherish them with much effort, and not waste them through forgetfulness and the like. To diligently practice, one should cultivate the perception of treating illness, just as a patient sees that if they do not take the medicine prescribed by the doctor, the illness cannot be cured, and so they take it. If one does not practice the instructions given by the Dharma speaker, one sees that one cannot subdue attachment and so forth, and therefore one diligently practices. One should not merely accumulate many categories of words without practice. Just as a leper with severed hands and feet will not be changed by taking one or two doses of medicine, we have been afflicted by the severe disease of afflictions since beginningless time, and it is not enough to practice the meaning of the instructions just once or twice. Therefore, in order to perfect all parts of the path, one should examine with discriminating wisdom and strive like the flow of a river. As the great teacher Chandra Gomi said in "Confession Praise": Even so, the mind is always deluded, and for a long time it has relied on disease. What can a few doses of medicine do for a leper with severed hands and feet? It is like that. Therefore, it is very important to have the perception of oneself as a patient. If one has this perception, other perceptions will also arise. If it remains only in words, one will not practice the meaning of the instructions in order to eliminate afflictions, but will only listen.

Therefore, just as a sick person who does not take the medicine prescribed by the doctor, but only focuses on compounding the medicine, cannot be liberated from the illness, it is said in the Samadhiraja Sutra: "People are sick, and the body is suffering, and sometimes they cannot be liberated for many years. This is because they are troubled by illness for a long time, and they also seek a doctor to cure the illness. Again and again they seek, and they find a doctor who is skilled in medicine. The doctor also, with a compassionate heart, gives the instruction, 'Take this medicine,' and takes many precious and good medicines, but does not take the medicine that can cure the illness. It is not the doctor's fault, nor the medicine's fault, but the patient's own fault. Likewise, for this teaching, although one has renounced, and knows all the powers, meditations, and faculties, one does not strive to practice. If one does not strive for reason, how can one be liberated from suffering?" It is also said, "Although I have proclaimed the excellent Dharma, if you hear it but do not practice it accordingly, it is like a sick person holding a bag of medicine, but unable to cure his own illness." Also, in the Bodhicaryavatara, it is said, "These things should be practiced with the body. What is the use of merely speaking? Does merely reading medical texts benefit the sick?" Therefore, one should generate the recognition of diligence as eliminating illness. The diligence spoken of here is to put into practice the points of what should be adopted and abandoned from the instructions of the spiritual friend. And to practice, one must understand; to understand, one must hear; and the necessity of understanding after hearing is also for practice. Therefore, it is crucial to put into practice each of the meanings of what is heard, according to one's ability. Just as it is said in the Chapter on Hearing: "If one hears a lot, but does not keep the precepts well, then one is blamed for the sake of the precepts, and one's hearing is not perfect. If one hears little, but keeps the precepts well, then one is praised for the sake of the precepts, and one's hearing becomes perfect. If one hears little, and does not keep the precepts well, then one is blamed for both, and one's conduct is not perfect. If one hears a lot, and keeps the precepts well, then one is praised for both, and one's conduct becomes perfect."
