The work is, the sign of entering in accordance with it, the colors are white and red. The ultimate mind of the union of skillful means and wisdom, whichever of the two Bodhicitta, there are no other complete dharmas, all the activities of the doctrine of benefiting beings, arise from that very door. The seed arose in the land of Hor (Mongolia), the sprout grew in the region of U-Tsang (Central Tibet), benefiting others in Kham and Mon (Bhutan). May all future, past, and present be auspicious with perfect abundance! The root of all diseases is ignorance, and the wisdom of knowledge that uproots it. Surpassing the conduct of ordinary medicine, I prostrate to you, the embodiment of the supreme physician. As long as this body exists, it has the characteristics of the form aggregate. When the three humors—wind, bile, and phlegm—are in balance, the nature of disease is latent, existing as if it has not transformed into a disease state. However, when these three are not in balance, diseases such as excess and deficiency arise. Moreover, because of the existence of this body, the suffering of illness is felt. The thought, "It is like this," is the feeling aggregate. The thought, "Now I am suffering from such a disease," and the continuous contemplation of the pain, is the perception aggregate. Past actions of various kinds, and wrong diet, clothing, medicine, and treatment, cause the disease to increase again, which is the formation aggregate. The great pervader of all these is the consciousness aggregate. Now, since you are a great meditator who has realized the nature of mind, you have examined the source, dwelling, and cessation of mind, but have not found it, and it is not established anywhere. Therefore, examine the source, dwelling, and cessation of disease as well. By maintaining naked awareness in a state where it is not established anywhere, if there is no body that thinks "I," and no mind that clings to "I," then the clinging to the form aggregate will cease, and the disease will be like a dream and an illusion. Similarly, instead of thinking, "I am suffering from such a disease," examine the mind of the one who is sick inwardly. In this way, the feeling aggregate that manifests the pain as disease will cease, and what cause is there for the mind to go into emptiness? When it ceases in this way, the perception aggregate, which takes the burden of continuous pain upon both mind and body, will be cut off.

Because the root of various thoughts arising from it is severed, the aggregate of formation also ceases, and there is no cause for sickness to arise again. That great self-nature of all things, consciousness, merely follows after thoughts and is subject to ignorance. Therefore, in the great self-luminous clarity of non-conceptual awareness, neither samsara nor nirvana, neither happiness nor suffering, will arise. What is called sickness does not exist in the knowable. As it says in the Uttaratantra: "The noble ones have completely abandoned the suffering of old age, sickness, and death." The meaning of this is not that even after the noble ones have taken a physical body, there will not be even a slight appearance of sorrow, old age, sickness, and death. Rather, it is because those who have realized the truth, in the samadhi of their own nature, are separated from the five aggregates, and even in the post-meditation state, they are not stained by the defilements of the eight consciousnesses and do not dwell together with the phenomena of samsara, that it is said in that way. Even so, what hope can ordinary people have for such a thing? However, merely recognizing the nature of mind and slightly holding onto emptiness makes a great difference compared to clinging to inherent existence. Therefore, do not add the burden of sickness to the mind, but sustain naked awareness in the midst of illness. Even if you die now, death is the great path for everyone. Turn your mind towards the sacred Dharma. Leave your homeland, meet a qualified lama, hear profound Dharma, and know the secret key of equal taste, so there is no regret. Even if you go to the next world, if those ignorant ones, for the sake of this life alone, are not afraid of the punishment of various weapons and are willing to be subjected to fire, water, and wild animals, then you should be happy now. Experiencing karma is the nature of things. Because in previous lives you accumulated heavy negative karma that would cause you to be born in the lower realms, now it ripens on this special body, thereby cutting off the root of experiencing retribution in other lifetimes. In the future, you will obtain a body in a particularly good rebirth, and the conditions for accomplishing enlightenment will be complete. This attainment is the compassion of the lama, the blessing of the Dharma, and the power of practice. Just as the armies of Mara surged forth when Shakyamuni Buddha was about to attain enlightenment at dawn, you should be happy now and strive in the equal taste! This was said by Jigme Tharpa of Dokham when he was afflicted by a severe illness, a Dzogchen practitioner.
