The explanation of "it is spoken in the realm of designation" is: By means of "if that exists, then it exists" itself, in the realm of the terminology of cause and effect, whatever speaks thus is shown, meaning, if the cause exists, then the effect will exist, this is "if that exists, then it exists" itself. Thus, by means of whatever exists, then it exists itself, that which is spoken as the effect is shown in the realm of the terminology of the two, cause and effect. The so-called "meaning is not different" means that here, cause and effect are different, and "if that exists, then it exists" itself is not different. The so-called "how is it?" is giving an example. Like understanding an ox from dewlaps etc. Meaning, just as an ox is understood from dewlaps etc.
