If stepping on someone's feet is inappropriate for a certain patient, then acknowledge that they are a patient. Therefore, they should take off their shoes, step on them with both feet, and then eat. The statement "Naked or wearing a single garment is not allowed" refers to the act of eating. The statement "A patient has no transgression" means that here, a patient refers to someone who is unable to do otherwise. Therefore, they have no transgression if they eat naked or wearing a single garment. The statement "They should be supported by attendants" means that the patient should be supported by attendants with their body and clothing, etc., so that they can eat. Here, the scripture is what the Bhagavan said: "One should not eat naked." Because the patient cannot tolerate combining, the Bhagavan said: "A patient has no transgression." "They should be supported by attendants with clothing." If able, one should rely on a dust-covering. If the patient is able, they should rely on a dust-covering. If able, one should not eat without the upper Dharma robe. The statement "The secret aspect is also" means that practitioners should rely on the final lower garment that covers the three areas. This should be understood as belonging to both the case of being naked and the case of wearing a single garment. The statement "Sitting for food, one should not let the beggar understand by saying 'Give this' or 'Give from here'" means that a person who is sitting for food and is prohibited should not say to the beggar, "Give this thing" or "Give public goods, etc., from here," and should not say it in a way that they understand. The statement "For a patient, what is suitable has no transgression" means that if the patient says, "Give me this food that is suitable for the illness and will balance the elements" or "Give from here," and makes them understand, there is no transgression. Here, the scripture is that for a patient, if that will balance the elements, there is no transgression. This is a summary of that point. In order to show the examples from some scriptures, for example, "Cook with low heat, moisten with high heat," it is said, "Cook with low heat, give this, give from here," and "Moisten with high heat, give this, give from here," and if they understand, there is no transgression. One should maintain the perception of being a patient and the perception of food being medicine, and then eat.

Hunger and thirst are diseases. Because I have them, I should cultivate the perception of myself as a patient. And because food nourishes, I should cultivate the perception of food as medicine, and then eat it. Also, remember the food, approach it, and eat it. How to remember it? Therefore, it is said that through arising, similarity, change, opposition, common cause, dependent appearance, seeking, dependence on others, association with disease, and separate disintegration, food should be regarded as impure. Therefore, these ten aspects, such as arising, should be combined with the impure recollection of food, and then eaten. Among them, arising means: this food arises from impure things and feces, etc. Similarity means: for example, rice porridge is similar to worms, flour is similar to bone meal, and so on. Change means: after eating food, it will turn into phlegm, snot, pus, blood, and so on. Opposition means: for the sake of food, it is associated with opposing things. Because of food, the mother fights with the child, the child fights with the mother, and so on. Common cause means: food has a common cause with extremely disgusting things such as feces and urine. Dependent appearance means: this arises dependently on conditions. This food arises depending on fields, etc., and depending on these, afflictions such as craving arise. Seeking means: it must be obtained through great effort. Dependence on others means: even if grains, etc., are obtained through great effort, if there is no cultivation, etc., it cannot be satisfied. Therefore, it becomes dependent on others. Association with disease means: because food is improperly digested, many diseases such as plagues arise. Separate disintegration means: this food will become four states, one part of which will be burned by the body's fire, the second part will become feces and urine, the third part will be eaten by worms, and the fourth part will cause the body to age.
