因为是地，所以对于这两者也应当认识到是粗大的。名为“用刀以外的”，意思是说，不能用刀以外的东西来切割痔疮。名为“那要用密咒和药物来治疗”，说的是那个痔疮。“不要因为厌恶而不去治疗”说的是痔疮。“为了治疗而在晚上吃东西是不如法的”意思是说，为了治疗疾病而在晚上吃食物，这不是不如法的，这两部经，即这部经和下面的经，是专门为了治疗的目的，这是所说的引申义。名为“不要怀疑对于这个未经加持的和储存过的东西是不可以的”，意思是说，不要怀疑对于这个治疗，未经加持的和储存过的食物是不可以的。这里经文是世尊所说的：“晚上也可以饮用。”因为没有加持的力量，世尊说：“如果没有加持的力量，也可以不加持而饮用。”等等。名为“为了治疗要吸食烟管的烟”，意思是说，为了做烟，用药物制作的烟管，那叫做烟管的烟，也就是从烟管里出来的烟，要吸食。这里经文是说：“因此允许吸食烟管的烟。”名为“那要用药管来完成”，说的是吸烟。“要用铁来做”说的是药管。“十二指（寸）长是好的”意思是说，药管的长度，十二指（寸）长是好的。“不要锋利和粗糙”说的是药 管的内壁。“用酥油或油麻油涂抹后放入鞘中”说的是药管。“那要放在针筒或法衣的挂钩上”说的是药管的鞘。“为了没有污垢要放在火上烧”说的是药管。“要做滴鼻子的工作”说的是吸鼻子的工作。“那要用滴鼻子的器具来完成”说的是滴鼻子的工作。“两个不锋利的嘴是好的”说的是滴鼻子的器具，是分开的两个不锋利的嘴

Because it is earth, therefore these two should also be recognized as being large. The phrase "other than a weapon" means that hemorrhoids should not be cut with anything other than a weapon. The phrase "that should be treated with secret mantras and medicines" refers to that hemorrhoid. The phrase "do not fail to treat it because of disgust" refers to the hemorrhoid. The phrase "eating at night for the sake of healing is not improper" means that eating food at night for the sake of healing a disease is not improper. These two sutras, this one and the one below, are specifically for the purpose of healing, which is the extended meaning of what was said. The phrase "do not doubt that unblessed and stored things are not permissible for this" means that you should not doubt that unblessed and stored foods are not permissible for this healing. Here, the sutra is what the Bhagavan said: "One should also drink at night." Because there is no power of blessing, the Bhagavan said: "If there is no power of blessing, one may also drink without blessing." and so on. The phrase "for the sake of healing, one should inhale the smoke of a pipe" means that for the purpose of making smoke, the pipe made from medicine is called the smoke of a pipe, that is, the smoke that comes out of the pipe, one should inhale it. Here, the sutra says: "Therefore, it is permitted to inhale the smoke of a pipe." The phrase "that is accomplished with a medicine tube" refers to inhaling smoke. The phrase "it should be made of iron" refers to the medicine tube. The phrase "twelve fingers (inches) long is good" means that the length of the medicine tube, twelve fingers (inches) long is good. The phrase "do not make it sharp or rough" refers to the inside of the medicine tube. The phrase "after smearing it with ghee or sesame oil, put it in the sheath" refers to the medicine tube. The phrase "that should be placed on a needle case or the hook of a Dharma robe" refers to the sheath of the medicine tube. The phrase "to be free from dirt, it should be put on the fire" refers to the medicine tube. The phrase "one should do the work of dripping into the nose" refers to the work of inhaling into the nose. The phrase "that is accomplished with the instrument for dripping into the nose" refers to the work of dripping into the nose. The phrase "two non-sharp mouths are good" refers to the instrument for dripping into the nose, which is two separate non-sharp mouths.
