Having spoken of the action, "here" means the action of tending to the sick. Whoever establishes the sign of ordination, that is the establishment of his sign. If, for the sake of ordination, one merely adjusts oneself with signs such as shaving the head, then he should be regarded as similar to an ordained person. Just as tending to the sick of an ordained person has been explained, so too should the tending to the sick of one who has established the sign of ordination be done in the same way. "When those who live together in pure conduct are sick, they should not be abandoned" means that if someone among those who live together in pure conduct becomes sick, the monks in the Sangha should not abandon him. Saying "those who live together in pure conduct" is to indicate what must be done. To explain how not to abandon, "If he has no attendant, then appoint from the lowest downwards" means that if that sick person has no attendant, then the monks in the Sangha should appoint an attendant. From whom downwards should one appoint? Therefore, it is said, "from the lowest downwards." "From the lowest downwards" means that if one attendant cannot accomplish all the tasks of the sick person, then appoint a second. If those two also cannot accomplish it, then from the third onwards, appoint from the lowest downwards of the Sangha. Here, the scripture is: If there is nothing at all, the Sangha should fully understand the internal situation, and one or two or very many attendants should be appointed, and even all the Sangha should attend to the sick. In order to examine the medicine, it is said, "Medicine that comes from the Sangha can only be used by those within." Medicine can be used. From where does it come? It comes from the Sangha. By whom? Only by the sick person, not by the attendant and so on. Here, the scripture is: Whatever medicine is obtained from the property of the householder who is naked, that should be placed in the Sangha's pharmacy, and the sick monks should enjoy it. That statement is summarized by this sutra. "If it does not exist, then have the attendant search for it" means that if the Sangha's medicine and the sick person's own medicine do not exist.

Then, at that time, the attendant should be sent to seek other patrons, that is the final conclusion. If it is not obtained, then the Sangha should give, which means, if the attendant is sent to seek other patrons, but that medicine is not obtained, then at that time, the Sangha's property should be used for giving. Here, mentioning medicine is symbolic, any object that can improve the patient's condition, because it is urgent, even if one does not have it oneself, sending someone to look for it cannot be found, if the Sangha has it, then the monks who are the Sangha should give all of it. If there is not, then use the Buddha's activities, even including offerings to statues, etc., which means, if the Sangha does not have it, then the Buddha's activities should be given. Offering to statues, etc., refers to offering decorations, etc., to statues and stupas, these can also be given, let alone other properties of the Buddha or the Sangha, this is the final conclusion. Here it is taught that, if there is nothing beneficial to the patient's condition in one's own possessions, then at that time, all the activities of the Sangha should be given. If there is still not, then all the activities of the Buddha should be given, do not regret this, it is taught. Therefore, the activities of the Sangha, such as offering drinks and hanging umbrellas, etc., which means, for that patient, if oneself and his family cannot do it, then the monks who are the Sangha should use the Sangha's property to make offerings of drinks and hanging umbrellas, etc., to the Three Jewels. Therefore, combining what was said earlier, "If it is not obtained," it means, if one does not have one's own property, then one should use the Sangha's property to make offerings, etc. If there are no Buddha's activities, it means, if there is no Sangha's property, then all the Buddha's activities should be done. If the property of those two (Sangha and Buddha) is taken, if they do not die and have money, they should repay it, which means, the property of those two, the Sangha and the Buddha, is taken, it should be repaid. Who should repay it? Therefore, it is said "he," which refers to the person who was the intermediary before. When should it be repaid? Therefore, it is said "if they do not die," which means, if they do not die, then at that time. What should be used to repay it? Therefore, it is said "if there is money"
