And, the Khenpo and the Acharya, and the same Khenpo, and the same Acharya, and those who converse intimately, and those who say "please come", and acquaintances, and friends, should take care of the sick, it is said. The meaning of the sutra is that those who live together and so on should take care of the sick, this is the concluding word. Those who converse intimately, means to make happy with intimate words and so on, thereby achieving happiness itself. Those who say "please come", means wherever, those who hold weddings together and converse frequently. Acquaintances, means those who are familiar with whom they live together in the same place, or walk on the same road, and so on, and know each other. Friends, means whoever, no matter who sees or hears what, merely not conforming, but feeling happy. Now, in order to show the order of taking care of the sick by those who live together and so on, it is said, "If there is no first caretaker, then the next one will take care." Who is the caretaker? It is the caretaker. In the previous sutra, those who live together and attendants and so on were pointed out, the first of these is the one who lives together. If the first caretaker, that is, the one who lives together, does not exist, then the next one after the one who lives together, that is, the attendant and so on, will take care of and serve that sick person. Thus it is declared: If that sick person does not have someone who lives together, then at that time the attendant will take care of him. If that attendant is also not there, then the Khenpo will take care of him. If the Khenpo is also not there, then the Acharya will take care of him. If the Acharya is also not there, then the same Khenpo will take care of him. If the same Khenpo is also not there, then the same Acharya will take care of him. If the same Acharya is also not there, then the one who converses intimately will take care of him, and so on, it should be arranged in order. Among all the Acharyas, the Acharya who teaches reading is more important. In order to eliminate the doubt that "he is not included here", it is declared here that the Acharya who teaches reading is also included. In this occasion of taking care of the sick, mentioning the Acharya, it should be understood that the Acharya who teaches reading is also included. Therefore, if the caretaker Khenpo is not there, then the Acharya who gives the residence will take care of him. If he is also not there, then the worker will take care of him. If he is also not there, then the secret instructor will take care of him. If he is also not there, then the one who makes him fully ordained as a monk

Therefore, it should be done. If not, ultimately, the chanting acharya should do it. So, when those who live together, etc., become ill at the same time, how should the sick attendant perform their duties? Therefore, it is said, if it is not possible to do it simultaneously, then say, "Where to stay and where to enter." The object of being together is simultaneously, that is, when the sick are simultaneously afflicted by illness. If the sick attendant cannot attend to the sick, then the sick attendant should attend to the sick in the dwelling. Here, too, it should be understood that the word "dwelling" is the word for dwelling, because the word "dwelling" is used for dwelling in the work to be done. If many dwellings become ill at the same time, which dwelling should attend to the sick? Therefore, it is said, "Where to stay and where to enter," that is, attend to the sick of the dwelling that requests and does things. This is taught, if many who need to be attended to become ill at the same time, and the sick attendant cannot attend to all who need to be attended to, then at that time, attend to the sick of the dwelling. If there are many dwellings, then at that time, attend only to the one who requests and does things. If at that time, another who needs to be attended to becomes critically ill, and there is no fear of danger to the life of that dwelling, then at that time, attend to the one who is critically ill. Sutra is called Sutra because the meaning is briefly stated, therefore it should also be understood that this is also a statement. If those who do things, those who secretly teach, those who are fully ordained monks, those who chant, those who live together, attendants, the same Khenpo, the same Acharya, those who converse, those who say to come, those who inquire, and those who are friends become ill at the same time, even if it is not possible to attend to all of them, then at that time, attend to the sick according to the order previously stated, that is, first attend to the Acharya. If not, then attend to those who live together. If not, then attend to the attendant. If not, then it should be understood that one should attend to the sick of the same Khenpo, etc., in order, until the end of the friends. In order to also show similar sick-attending rituals to others, the sign created here is also similar to ordination.
