Therefore, the attendant should be the one who resides at the end. If that is not available, then the Khenpo should do it. If that is not available, then the Acharya should do it, and so on. Because the Khenpo is more important, before the Khenpo, the one who is most suitable should be the Acharya who resides at the end, along with the monastery, to be the attendant. Therefore, because he is a very close attendant, the rest should be the first to be suitable. Here it says, "Also accept the reading Acharya," which means accepting the Acharya. If it is not possible to do it all at once, then who should be relied upon, and who should be entered into? "All at once" refers to the many attendants of the patient. "Not possible" means that if one person cannot be an attendant, then one should be close to the one who can be an attendant. Who should be relied upon? It is said, "Who should be entered into?" That is, whenever one requests and does something, for whom, what is more important, one should enter into that person's sphere. One should understand through meaning that "for the poverty that causes great harm, one should abandon everything." Here it says, "Even if it is just his symbol, it is similar to ordination," which refers to the place where the patient is being cared for, for those who have shaved their heads, etc., for the purpose of ordination, but are only combined with symbols, but are not yet allowed to be ordained, one should regard them as similar to ordination. Just as one treats an ordained person, one should also treat the patient in that way, that is its meaning. One should not abandon those who practice equally with Brahma. If he does not have a patient, then one should appoint from the lowest. If he does not have, it means together with the monastery, etc. From what below should one appoint? Therefore, it is said, "the lowest," that is, if one person is given, but he becomes without an attendant, because he does not have the ability to obtain the fruit, then another should be appointed. Similarly, until the lowest of the Sangha, one should appoint from them, that is its meaning. The text here is that if everything is not available, one should fully understand the situation of the patient in the Sangha, one or two or very many patients should be arranged, and even the entire Sangha should take care of the patient. In order to examine the medicine, the medicine obtained from the Sangha is only for the patient.

It is said that it can be used, etc. Whatever medicine is among the items donated by the householder, it should be placed in the Sangha's pharmacy, and the sick monks should enjoy it. This is what this sentence summarizes. If there is really none, then the attendant should be sent to find it. It means that if there is none, the attendant should be sent to find this medicine. If it cannot be found, the Sangha should give alms. It means that if even after sending someone to find it, it cannot be found, then the Sangha should give alms. "None" refers to the Sangha's. Even offerings to Buddha images, etc., should be given. It means that whatever is made, such as Buddha images, stupas, and decorations, even these things should be given. What need is there to mention others? Therefore, the Sangha should make offerings such as drinking water and holding umbrellas. This should be understood in connection with "if it cannot be found," etc. If one does not have it oneself, one should ask the Sangha for help. If the Sangha does not have it, one should ask the Buddha for help. If these two have taken it and that person receives it before dying, it should be returned in full. The "these two" refers to the Sangha and the Buddha. If there is no almsgiving, there is no fault. The attendant should not fail to serve him. The "him" refers to the patient. One should not break words and deeds that are in accordance with his words and meanings and in accordance with the Dharma. The "in accordance with meaning" refers to being beneficial to life and being correctly related to each other. "His" refers to the patient. "In accordance with the Dharma" refers to being beneficial and related to merit. If the patient asks for a particularly cherished item, one should not refuse. The "particularly cherished item" refers to things like bowls to which the patient is particularly attached. "Asking" refers to the patient asking the attendant. One should not advise, which refers to the attendant advising the patient. The patient should not disobey, meaning that the patient should not disobey the attendant's advice. If the attendant is worried that the patient will die, he should be helped onto the Sangha's mattress and placed in an individual's position. Massage and bathing should be done in advance, which is a skillful means.
