Stones, malachite, epilepsy, beriberi, wasting disease, emaciation, forgetfulness, hunger, tinea versicolor, foot swelling, nocturnal emission, plague, severe infectious disease, one-day fever, two-day fever, three-day fever, four-day fever, combined fever, daily fever, constant fever, relapsing fever, scabies, measles, hiccups, phlegm, difficulty breathing, swelling, limb stinging, tumors, blood disease, bile disease, hemorrhoids, nausea, edema, fatigue, body fever, rib fever, bone stinging. Do you not have any of these kinds of physical illnesses, or other similar illnesses, in your body? He should answer, "No." Venerable one, listen! What I am asking you now, the learned practitioners of pure conduct in the Sangha will also ask you. There, too, you should not be afraid, say yes if it is yes, and answer no if it is no. Do not come until you are summoned. This is the secret instruction. After that, the monk who gives the secret instruction stands next to the elder, and with folded hands, says this: Please listen, venerable Sangha. If my preceptor, named so-and-so, secretly points out the obstructive dharmas to the one named so-and-so, then these obstructive dharmas will be completely purified. If it is indeed so, what does it matter? Everyone should say, "If it is completely purified, that would be good." If they say that, then it is good. If they do not say it, it will become a serious offense. This is requesting to come. After that, the one to be fully ordained is brought to the mandala and made to prostrate to the elders. The officiating monk should have him request full ordination. He should have him request in this way: First, have him prostrate to the teacher. Having him join his palms, he should say these words: "Venerable Sangha, please listen. I, named so-and-so, state my name for the sake of meaning. Relying on the preceptor named so-and-so, I desire to receive full ordination from the Sangha. I, named so-and-so, state my name for the sake of meaning, relying on the preceptor named so-and-so, request full ordination from the Sangha. I beseech the venerable Sangha to grant me full ordination. I beseech the venerable Sangha to consider me. I beseech the venerable Sangha to accept me. I beseech the venerable Sangha to instruct me. Compassionate venerable Sangha, for the sake of compassion, please be compassionate to me." Repeat this twice or three times. This is the request for full ordination.
