The six stains, the five obscurations, the five that correspond to the lower part, the five that correspond to the upper part, the ten manifold grounds, the two non-virtuous manifold grounds, and the fifty-seven minor bases of yogis. There are many near afflictions, but mainly, as they are, there are one hundred and fifty-six. That which is called "even one of these," out of all the hundred diseases, even one anger can destroy all the beings dwelling in the world. For example, as it is said elsewhere, like a sage being angry. These increase due to the harm of craving and anger. One obtains great faults due to delusion, such as it is said elsewhere, like doubt is a great fault, and so on. These, that is to say, the other medicines for curing the causes of the hundred diseases that arise in the body, even if sought from all directions, cannot be found, and one cannot cure epidemics and so on. Among those that are not sudden, there are one hundred and one that arise from wind, and likewise, those that arise from phlegm, those that arise from bile, and those that arise from a combination. These four hundred and four diseases are those that are spoken of in the "Questions of Arya Daana" and in the "Vinaya of the Great Assembly of Holy Ones." Because even one disease can transform and destroy all the beings dwelling in Jambudvipa, the Lord saw that the one who had become the master of the land of Kashi was weakened, and asked the doctor. Since the medicine for it could not be found in any direction, and thinking that it was impossible, the doctor showed a large piece of fresh meat, and he cured it by accomplishing the waves of truth. Being afflicted by these four hundred and four diseases is because one is afflicted by the causes of these diseases. Therefore, their doctor is the all-knowing one, because the all-knowing doctor removes the cause of physical suffering and causes the removal of suffering. For whoever possesses the nature of removing all suffering, that is the one who removes all suffering. Not practicing or contemplating the instructions of the medicine of the Dharma, that is, the supreme Dharma, is a place of extreme stupidity and reproach.

It is truly born of extreme ignorance. Since that is the case, even at a small, ordinary cliff edge, one must remain cautious. Even a person suffering from a minor illness should not disregard the doctor's advice. How much more so when falling into a thousand-yojana-long cliff edge? Regarding the diseases of the mind, how can one not be cautious, let alone disregard the teachings of the all-knowing physician? Even at a small cliff edge, carelessness will lead to great disaster, this is a truth that must be understood. Even if that is the case, thinking, "I will not die today," and remaining comfortably for the time being, and then confessing sins and performing virtuous deeds later, saying, "I will not die today," and staying comfortably is not right. When impermanence arrives, one will surely be devoured by demons. Do not be unafraid of sins, for they are bound to happen. To illustrate this point, it is said: "Who can grant me fearlessness from death? How can one be liberated from it? If one cannot escape being devoured by demons, why remain comfortably?" If one thinks that one can be comfortable through enjoyment, then listen: The enjoyments that have been experienced in the past have already vanished, what remains? I am attached to enjoyment and have disobeyed the guru's teachings. If one thinks that one can be comfortable by not understanding the inner friend, then listen: When alive, one will likewise abandon relatives and friends and have to leave alone, then what is the difference between friends and non-friends? Because one cannot do anything for them. Suffering arises from non-virtue, how can one be liberated from it? Day and night, I should only contemplate this. This is saying, by what method? It means that if one is not liberated oneself, one cannot help others to be liberated. To illustrate that confessing sins and so on are methods for escaping suffering, it is said: I am an ignorant ordinary person. Because of ignorance, I do not know what is beneficial and what is harmful, I do not know what to acquire and what to abandon. Whatever sins have been committed by body, speech, and mind.
