"The old thumb of the mantra practitioner" refers to the old thumb of the mantra practitioner Vajradhara, which is the head of the five fingers, he is the common master. The meaning of the inseparability of the three Vajras, Vajradhara "Ma Ma" is clear. Repeating it twice is the sixth Vajradhara.
