Thinking about whether a severe illness can be cured, one looks to the stars. Among them, if the stars Mars, Venus, Snarma, Yama, Wet Star, and Kang are afflicted by disease, it is difficult to cure. Again, one thinks, even if the stars are bad, one must look at the time. Among them, if afflicted by disease in autumn, winter, at sunset, at midnight, or at moonset, it is difficult to cure. Again, one thinks, although many of these omens are bad, perhaps it can be cured, perhaps it cannot. One must look at the patient himself. If he is a person with merit, he can be cured. If he has no merit, what good are good omens? Thinking this way. Setting out on the road with the messenger, one thinks again, if the patient has signs of longevity, he can be cured. If one encounters signs of a short life, it cannot be cured. Thinking this way, on the road, one sees two small boys pulling and fighting each other, tearing at each other's hair, throwing mud, stones, weapons, and sticks, or one sees a torch extinguish itself, or one sees someone cutting down a tree, or one sees someone walking on the road mending leather in his hands, or one sees items left on the road, or one sees someone carrying an empty container, or one sees a solitary monk walking without companions, or one sees a tiger, a wolf, a crow, a firefly, a fox, and so on. Again, one thinks, from the dispatched messenger to everything seen on the road, if everything is inauspicious, then it will be difficult to cure the patient without being harmed by spirits. Again, one thinks, if I do not go, I will not be known as a skilled doctor, but if I go, I will not be able to cure him. Thinking this way. Again, one thinks, although there are so many inauspicious omens, one should still go to the patient with single-mindedness. Thinking this way, it is not good to hear these sounds on the road. Which ones? Hearing the words death, loss, occurrence, breakage, collapse, rupture, detachment, falling, liberation, carrying, burning, and future, it is difficult to cure, and one cannot protect. Also, if one hears the sound of a bird from the south, such as the sound of a crow, a firefly, a cuckoo, as well as the sound of a dog, a mouse, or a fox.

When hearing the sounds of a rabbit or a pig, one might think, 'This patient is very difficult to cure.' Then, upon entering the patient's home and observing the patient, sometimes they are cold, sometimes hot, and all their joints ache intensely. Their eyes are red and tearful, there are sounds in their ears, their throat is blocked and painful, their tongue is cracked, their complexion is very dark, they cannot lift their head, their body is emaciated and dusty, their urine and feces are blocked, their body suddenly becomes very fat, very white, and red. The sound of their speech is uneven, sometimes thick, sometimes thin, and their whole body becomes mottled, with strange colors of blue and yellow appearing. Their abdomen is full and swollen, and their speech is unclear. Having seen them in this state, one asks the patient's attendant, 'What has changed with this patient from before?' The patient's attendant says, 'Great doctor, this person used to have faith and respect for the Three Jewels and the gods, but now they have changed and have no faith. They used to enjoy giving, but now they have become miserly. They used to eat little, but now they eat a lot. They used to be gentle and honest, but now they have become angry and full of hatred. They used to be attentive and show great respect to their parents, but now they have no respect for their parents.' Having heard these words, the doctor approaches and smells them. Do they smell of lotus? Or of agarwood and various incenses? Or of spices, cardamom, nutmeg, cinnamon, saffron, and sandalwood? Or do they smell of roasted meat? Or of wine? Or of tendons and burnt bones? Or of fish and foul vomit? Having discerned what pleasant or unpleasant smells are present, one smells the body. Is the body like silk? Is it soft and smooth like a lotus flower? Or is it hard like a stone? Is it cold like ice? Is it hot like fire? Is it rough like sand? Then, that experienced doctor, having seen such various signs, knows that the patient will die without being consumed by demons, but does not utter the definitive words, 'This person will die,' and says to the attendant: 'This...'
