Those many retinues, having offered various kinds of offerings to the Bhagavan, generated the mind of unsurpassed, perfectly complete enlightenment. Immeasurable and boundless Bodhisattvas, as many as the sands of the Ganges River, were abiding on the first ground. At that time, the Bhagavan prophesied to Manjushri, the youth, the Bodhisattva Light Holder, and Tsunda, and spoke these words: 'Sons of good family, be very diligent in your minds towards me, be mindful! Now, because of a backache, I am in pain all over. You should think that I am lying down as if in pain like a small infant. Manjushri and the others, extensively teach the Dharma of the Great Vehicle to the four assemblies. Now, I entrust all these Dharmas to you. Likewise, Light Holder and Joyful One, I also entrust these pure Dharmas to you.' At that time, having spoken these words, the Thus-Gone One, in order to tame each individual being, lay down on his right side like a sick person. Then, the Bodhisattva Light Holder said to the Bhagavan: 'Bhagavan, the Thus-Gone One is free from all faults of suffering from illness, and has no weariness. Bhagavan, all sentient beings become the source of illness due to the four kinds of fierce thorns, namely, desire, hatred, ignorance, and pride. If there is a cause of illness, many illnesses will arise, namely, fever from desire, coughing, shortness of breath, nausea, numbness of the whole body, mental confusion and unconsciousness, alternating fever and chills, measles, hot and painful eyes and ears, stiff back, abdominal distension, madness, emaciation and dryness, being deluded by ghosts, and so forth. Since the Bhagavan does not have various illnesses of body and mind, why then does the Thus-Gone One now show his life, saying that I have a backache, and Manjushri, you teach the Dharma to the many retinues?' One will not suffer from the two kinds of illnesses, cause and condition, namely, loving-kindness and compassion towards all sentient beings.

I prostrate. To the sick, giving medicine for the cause of illness and giving to the poor. The Tathagata, in the past, for countless eons, was extremely diligent on the path of Bodhi. He constantly brought happiness to sentient beings with pleasant words and did not turn to suffering. He gave various medicines to the sick. Why is he sick like this now? World Honored One, the sick in the world sometimes sit and sometimes lie down. Some, due to discomfort, beg for food and drink. Some teach their families how to manage their livelihoods and work. Why does the Tathagata not speak a word and sleep? Why does he not teach the Shravaka attendants the Paramita of discipline, Samadhi, liberation, Samapatti, and diligent abandonment (of evil deeds)? Why does he not teach these? Why does he not speak these extremely profound Mahayana Sutras? Why does the Tathagata not, with immeasurable means, teach the great light protector (Sanskrit: Mahārakṣita), who is like the king of elephants among men, and the great beings, that they will not regress from Anuttara-samyak-sambodhi? Why does he not punish those evil Bhikshunis who hold all unexamined impure objects? World Honored One, clearly not sick, why does he not speak a word and lie down on his right side? When Bodhisattvas give medicine for the illness to the sick, they dedicate all the roots of virtue to sentient beings and dedicate them together to the omniscient wisdom, thereby completely eliminating all sentient beings' afflictive obscurations, karmic obscurations, and resultant obscurations. The so-called afflictive obscurations are greed, hatred, ignorance, and the afflictions caused by anger, jealousy, stinginess, deceit, hypocrisy, shamelessness, immodesty, arrogance, excessive pride, pride beyond pride, conceit, false pride, superior pride, inferior pride, carelessness, haughtiness, resentment, contention, and evil.

Greed and, feigning with deceit, seeking gain with gain, seeking with what is not Dharma, seeking in abundance, having no shame, not acting according to the teachings, relying on evil friends, not being content with attachment to gain, being bound by bonds and difficult to liberate, desiring evil desires, being attached to evil attachments, viewing the destructible as self, clinging to existence, clinging to non-existence, being lazy and distracted, joy and, sleep and, yawning and, displeasure and, attachment to food and, drink, dullness of mind, focusing the mind on other things, thinking of the non-virtuous, acting badly in body and speech, delighting in much talk, the senses being dull, speaking all sorts of lies, being completely covered by thoughts of desire and, thoughts of hatred and, thoughts of harm, this is called the obscuration of afflictions. And the karmic obscuration of the five inexpiable sins, and being born in the hells of sentient beings and, animals and, hungry ghosts and, slandering the holy Dharma and, being born as a heretic, this is called the obscuration of ripening. These three obscurations are called great diseases, and Bodhisattvas, while practicing the conduct of enlightenment for countless eons, gave the gift of medicine to all the sick, and made this aspiration: May all sentient beings be completely free from disease, and may they obtain the Vajra body of the Thus-Gone One. May I become the supreme king of medicine for all immeasurable sentient beings, and may I dispel all incurable diseases. May all sentient beings obtain the medicine Agada, and by the power of that medicine, may they dispel all harm. Furthermore, may all sentient beings not turn back from the Unsurpassed Perfect and Complete Enlightenment, and quickly find the medicine of the Unsurpassed Buddha, and may they dispel all the thorns of afflictions. Furthermore, may all sentient beings, by exerting great effort, possess the mind like the Vajra of the Thus-Gone One, and having become the supreme medicine, may they heal all diseases, and may they be without the thought of strife. Furthermore, may sentient beings become great trees of medicine, and may diseases not
