A hundred trillion beings appeared, and each of these beings extended a hundred hands, offering incense, garlands, and various perfumes to the Tathagata. The Bhagavan said: Medicine Deity, do you see these beings offering incense, garlands, and perfumes to the Tathagata? He replied: Bhagavan, I see them. Sugata, I see them. The Bhagavan said: These beings that have appeared are without mind. These people are without mind. Then, each of those hundred trillion people, all of whom had a hundred hands, all fell down. Then, the Bodhisattva Mahasattva Medicine Deity, having seen this, said to the Bhagavan: Bhagavan, what is this? Sugata, what is this? Why do the hundred hands of these people fall off so quickly? Bhagavan, if a hundred hands do not fall off, what need is there to mention people with only two hands? The Bhagavan said: Medicine Deity, in the same way, sentient beings are born without mind and cease without mind. Medicine Deity, know that their bodies also become without mind. He replied: Bhagavan, among these sentient beings, which are the young and which are the old? The Bhagavan said: Medicine Deity, there are young sentient beings and there are old sentient beings. He replied: Bhagavan, which are the young and which are the old? The Bhagavan said: Those that have fallen now are the old, and those that have grown from the trees are the young. He replied: Bhagavan, I wish to see the young sentient beings. Then, the Tathagata Moonlike One extended the palm of his right hand, and from the ten directions came a hundred trillion beings. From the lower and upper directions came fifty-five billion beings, and those beings gathered and bowed their heads at the feet of the Bhagavan, not speaking to the Tathagata, not speaking at all, and sitting without saying anything. Then the Bodhisattva to the Bhagavan...

The Bodhisattva Mahasattva Medicine Deity (sman gyi sde) asked thus: "Venerable One, why do those sentient beings not speak to the Tathagata, always remaining silent and not uttering a single word?" The Venerable One replied: "Medicine Deity, do you not know? The earth has no mind, so it does not speak, always remaining silent and not understanding the Dharma. Why is that? Medicine Deity, because some young sentient beings here, even when they see, do not understand birth, cessation, aging, sickness, sorrow, lamentation, suffering, separation, displeasure, encounter, friendship, separation, death, untimely death, and all unbearable sufferings. Even if they see, they will not become disgusted, so how can they understand? Medicine Deity, they should be taught repeatedly." Then, the Bodhisattva Mahasattva Medicine Deity asked the Venerable One thus: "Venerable One, where do those young sentient beings who do not seek the Dharma come from? From where do they die, and where will they be reborn?" The Venerable One replied: "Medicine Deity, listen! Those who obtain a human body are not made by a silversmith, nor by a coppersmith, nor by a carpenter, nor by a potter, nor by the fear of a king, but rather they arise from the union of men and women and from sinful deeds. These sentient beings also experience immeasurable and unbearable suffering and feelings by repeatedly learning skillful crafts. There, they experience the ripening of the evil and unwholesome deeds they committed in the past. Medicine Deity, here, those young sentient beings who cannot endure the cold will experience such painful feelings. Medicine Deity, therefore they do not speak, always remaining silent. Medicine Deity, in this way, those young sentient beings neither understand good, nor do they understand birth, nor do they understand cessation. Those who cannot obtain a human body are also called Medicine Deity young sentient beings." Asked: "Venerable One, how do young sentient beings arise? How do they cease?" The Venerable One

The Buddha of the past declared: The category of medicine is like this, for example, when someone touches fire with wood, that wood will eventually be ignited by the fire. The category of medicine initially arises in the human body, and after arising, it first experiences feeling. Please tell me, World Honored One, who is it that manifests birth here? And who is it that, after being born, is completely liberated from suffering? The World Honored One declared: The category of medicine is the Buddha himself being born, the Tathagata himself being completely liberated from suffering. It is like this, for example, when a king binds someone and imprisons them in a dark room, that person, upon entering the dark room, sees the dark room as a dark room. Then, another person, who has previously been touched by the feeling of suffering, sees that person and becomes miserable. Having not experienced suffering before, life will come to an end. After thinking this, he brings fire there and throws a little fire into the house, and the imprisoned person sees that fire. Upon seeing it, he is relieved and feels joy. That fire, burning for some reason, causes the entire house to burn, and that person is burned to death as a result. When the king hears that the person has been burned to death, he is very unhappy and thinks: From now on, in my country, no sentient being will be imprisoned. Then, that king says to the sentient beings residing in his country: Hey, sentient beings, do not be afraid! Do not be terrified! Do not be frightened! From now on, in my country, there will be no punishment by execution, nor will anyone be imprisoned. No sentient being will be deprived of life. Hey, sentient beings, do not be afraid! Just like that sigh of relief, the category of medicine, the Tathagata, is the one who burns away all afflictions and completely pacifies all diseases. Just as when a house is burned down, the person's body is also burned, entering for the sake of the benefit, good, and happiness of sentient beings, and liberating bound sentient beings completely from their bonds. The Tathagata also abandons the defilements of desire, hatred, and ignorance, and appears in the world like a lamp, for the minds of sentient beings.
