The first is the four lines beginning with "Grasping and Grasping Dualistic Thought." "Grasped" refers to the inverted attachment to external objects such as forms and sounds, both internal and external. "Grasping" refers to the inverted attachment to the consciousness of the senses, such as eye consciousness. In the city of the body, which is the basis where these dualistic thoughts reside, there are impure substances such as excrement and urine, as well as flesh and bones (earth element), blood and pus (water element), warmth (fire element), and breath (wind element). At the time when these four elements and their aggregate, this illusory aggregate of aggregates, separate from one's own consciousness, and various causes of death such as improper food arise, please bless and pacify the suffering of severing the vital points. This is the first part. The meaning of "city" is explained according to the confusion of grasping and grasped dualistic thoughts. As in the Sutra of the Mother, the sense organs such as the eyes are called "villages," and the body, which is the basis of the senses, is called "city." Therefore, the city refers to the basis of confused consciousness, the general and specific elements of the body. Between "impure four elements," adding a six-syllable phrase with a common basis to refer to the four elements as impure seems barely justifiable. The second is the four lines beginning with "Cherished and Protected." This body, which was previously cherished and protected with food and clothing, deceives oneself when it is needed later, not performing activities such as eating and digesting, and losing the ability to serve as the basis of consciousness. At that time, the extremely terrifying enemy, the messengers of Yama, appear and begin to lead one to the other side, and when the three poisons of desire, hatred, and ignorance, and the separation of the elements arising from their influence, sever one's life with the sword of intense suffering that cuts off the vital points, please bless and pacify all the various bad illusions arising from inauspicious circumstances. This is the second part. Most people afflicted with cold diseases do not experience hallucinations at the time of death, but most people with severe fever do experience hallucinations. Therefore, some extremely sinful people may not experience hallucinations and intense suffering at the time of death, while virtuous actions are powerful.

It is possible that some who have done good deeds may encounter such situations. Alternatively, the former is because one must experience great suffering in the lower realms in the future, so it has not happened yet; sometimes, small fruits of virtue arise. The latter is because one will experience happiness for a long time in the future, so for the time being, the remaining fruits of past misdeeds are mostly offset by experiencing small sufferings. As stated in the "Perfection of Wisdom in Three Hundred Lines" and others, do not dwell on the fact that some who have done great good deeds experience illusions and intense harm; and do not consider it good if some evildoers do not experience these things. Therefore, the ripening of karma is inconceivable, and one should not hastily judge others as good or bad. As the sutra says: 'I or someone like me can measure a person, but a person cannot measure another person, for they will err.' And, 'The ripening of karma is inconceivable; even with great compassion, one may be born as an animal.' Just as bodhisattvas may be born in the lower realms. Third, 'The doctor abandons,' etc., four lines: When the time of certain death arrives, the doctor knows that one cannot be saved with medicine and abandons and leaves. Moreover, even if great rituals are performed, death cannot be averted. When relatives think, 'I am certainly going to die,' they give up hope of survival and suffer. The patient himself also feels helpless, not knowing what to do, as if helpless or ignorant. At this time, please bless me to remember the instructions previously taught by the lama, especially the special instructions for the time of death. Although there are many instructions, the most important are strong faith in the Three Jewels and the lama, not generating evil thoughts, not being attached to one's possessions, confessing sins, and remembering the instructions for transference of consciousness, etc. Fourth, 'Accumulated through miserliness,' etc., four lines: In the past, for one's own sake, through various efforts, one has accumulated food and wealth through miserliness, but when one goes to the next world, these will be left behind. One will be completely separated in a way that one can never meet again with relatives and others who love and care for oneself.
