Yellowish-brown tiger-striped one and the one born from a tree, And the four sages such as Caraka and others. The four medicine goddesses who gather the essence of the four elements, I prostrate and praise them and their retinue. Please clearly and without error, Show me the diseases, causes, and remedies of beings. This is the sequence of self-generation and approach. In the four intermediate directions are the four goddesses on sun discs, Exceedingly beautiful, adorned with silk and jewels. They hold vases of medicine essence and nectar. From the three syllables at the three places of all, Light radiates, inviting the wisdom beings. At the end of the syllable Om (ōng), the string of mantras rotates to the right, radiating light, offering to all the noble ones, and eliminating the suffering of all beings. All the realizations of samsara and nirvana, the light gathers and dissolves into the vase deity. From the body of the vase deity, the accomplishment of realization, its essence, nectar, descends. The vase is full, the water boils, returning and dissolving into the crown of my head. The sins and defilements of body and speech are purified. The essence of nectar dissolves into the heart, and the accomplishment of realization is attained. Recite 'Namo Om Vajra Bhaisajye Maha Bhaisajye Samungate Hum (namo oṃ vajra bhaiṣajye mahā bhaiṣajye samungate hūṃ)' about a thousand times. Visualize the eight attendants of the vase deity as described above. Recite 'Om Shiti Ghana Ghashi Susu Ricā Mayona Khri (oṃ śīti ghana ghaśi susu ricā mayona khri)', 'Sori Ni Guli Sumala (sōri ni guli sumala)' about a thousand times, and then practice. Mentally transform the real offerings, as well as the divine objects and pure objects of desire, and offer them to the Bhagavan vase deity. Please enjoy them and grant the accomplishment of wisdom. Recite 'Om Maha Bhaisajye Saparivara Rupa Shabda Gandhe Rasa Sparshe Dharma Pratīccha Svaha (oṃ mahā bhaiṣajye saparivāra rūpa śabda gandhe rasa sparśe dharmāḥ pratīccha svāhā)'. May this place be filled with the seven treasures, completely filled by the wise, and in order to grant accomplishments and wishes, may it be offered daily by the wise. Recite 'Om Maha Bhaisajye Saparivara Chakra Mani Stri Purusha Hasti Turanga Khadga Ratna Pratīccha Svaha (oṃ mahā bhaiṣajye saparivāra cakra maṇi strī puruṣa hasti turanga khaḍga ratna pratīccha svāhā)'. He is the teacher of gods and humans, the king of medicine, the Bhagavan is the protector of the three realms, the Medicine Buddha who cures the disease of the three poisons, I prostrate to that light of lapis lazuli. I prostrate and praise the four sages who manifest realization in the four directions, the four goddesses who manifest wisdom in the four intermediate directions, the assembly of Medicine Buddhas who eliminate the suffering of beings, and their retinue. For the benefit of all sentient beings, may the causes and conditions of diseases, diagnosis and treatment methods, like images in a clear mirror, appear in my mind, please bless me. May you not forget your past aspirations and vows, and accomplish the tasks entrusted to me. Next is the preliminary visualization, to perform the great practice: From emptiness, the mandala in front is a vibrant herb garden, delightful.
