I, named so-and-so, from this time forth, until I reach the heart of enlightenment, take refuge in the supreme of the two-legged, the perfectly complete Buddha, in order to not cut off the lineage of the Buddha. I take refuge in the one who possesses great compassion, who is all-knowing, all-seeing, free from all fears, the great being, the supreme among humans, whose body is inconceivable, the unsurpassed body, who possesses the Dharma body. I take refuge in the Dharma, the supreme that is free from attachment, which is possessed by the unsurpassed Tathagata, the peaceful Dharma body, and the wisdom of individual self-awareness. I take refuge in the Sangha, the supreme of the disciplined assembly, the noble Sangha of great Bodhisattvas who are irreversible from the four directions. Thus, three times. May I cause those who have not attained perfect and complete Buddhahood to attain perfect and complete Buddhahood. May I become a refuge, a dwelling, and a friend to the world that has no refuge, no place to go, and no friend. By this merit, may I attain the state of all-seeing, defeat the enemies of wrongdoing, and deliver beings from the great, turbulent waves of aging, sickness, and death, from the ocean of existence. I will follow in the footsteps of the Bodhisattvas, great beings, endowed with great compassion, who have appeared in the past, future, and present, who have correctly entered the Great Vehicle, and who abide on the great earth. Holy ones, please accept me as a Bodhisattva. May I be like the earth, water, fire, and wind, like grass and the trees of the forest, may I always be used by all living beings as they wish. May all Buddhas, Bhagavat, abiding in the ten directions, accomplish the intention to perfect the prosperity of myself and others. May all the palace mandalas be covered with the clouds of offerings that increase the practice of goodness. May the holy Dharma flourish and spread in all worlds through all means, and may it abide in the minds of all beings through writing, offering, reading, reciting, giving, teaching, listening, thinking, and meditating. May all Bodhisattvas fully accomplish their respective liberations and quickly attain the state of omniscience. May they enter the various Buddha-fields without hindrance, make offerings to the Tathagatas, accomplish the teachings correctly, and be endowed with the power to completely accomplish the immeasurable benefit of sentient beings, without lacking any resources, skillful means, and perfect power. May all the noble Pratyekabuddhas and Shravakas perfect their respective liberations, enter and practice the gate of the unsurpassed vehicle, and quickly attain the state of omniscience. May all the protectors of the holy Dharma completely ripen their own retinues, possess the power and glory to subdue opposing forces, be endowed with perfect skillful means, obtain the power to fulfill all great vows and spontaneously accomplish them, quickly perfect their respective manifestations, and quickly attain the state of the great Dharma Lord, the Buddha Bhagavat. May all the gods and protectors who unfailingly protect and benefit so-and-so's paternal family and their retinue, be free from all sins and faults of body and mind, and be endowed with the perfect glory of this world and beyond, and all good qualities. May they have unwavering faith in the three supreme jewels, completely accomplish the generation of the Bodhicitta, and gradually and quickly attain the supreme and holy state of Buddhahood.
