The body is majestic. The hair is upright, and the braid spirals to the left. Hairline adorned with snakes, wearing a garland of skulls. Behind the ears, flowers are inserted, adorned with ribbons. Drunk on blood wine, eyes red. Chewing on human corpses, revealing four fangs. Extremely wrathful, greatly delighted in blood. From the right ear hangs a snake, jingling. Taming the wicked, proclaiming the sound of Brahma. The left ear ornament is a red lion's mane. The hero roars like a lion in the three realms. Four hands, the first right hand holds a sword. The lower one holds a red pool of bhanda (Tibetan, Devanagari, Romanized Sanskrit, literal meaning). The first left hand holds the weapon of Shakti. The lower one raises a silk banner on a trident. With Shakti, swords, and weapons, those killed. A necklace made of the skulls of Asuras. The upper body wears a raw elephant hide. The lower body wears cut raw cowhide. Joyfully embracing the body of the Great Ishvara. When going to the four places of the Thirty-Three. The gods and Indra pay homage. When destroying the Asuras. All the lords of the Asuras pay homage. When the supreme body collides with Mount Meru. The sun and moon rise from the direction of the navel. The Four Great Kings and others pay homage. Able to ignite flames from the bottom of the ocean, able to dry up the sea. All the lords of the underworld pay homage. O Goddess, by displaying these miracles. Causing demons and heretic kings to tremble and submit. Causing the hosts of dakinis and rakshasis to be fearful and laugh. A precious tent is erected. From the Cittā's palace. Having gathered, they dissolve into the mandala of the sun. The poisonous vapors from the black direction, cause the four hundred and four diseases to arise. Therefore, the mistress of disease solemnly vows. Having gathered the essence of nectar from the ten directions of the world, they dissolve into the mandala of the moon. The nectar vapors from the white direction, eradicate the four hundred and four diseases. Nectar is the king of medicine, solemnly vows. Whoever has faith in you, is as loving as a mother. Therefore, you are the only mother, the mistress of disease, the lord of medicine. I always praise you, goddess. In the presence of Buddha Shakyamuni, from now until the end of samsara, in order to pacify diseases and sufferings etc. of the white direction, increase life and prosperity. Attract and control the black direction. Accomplish the deeds of madness, plague, killing, etc. in order to protect the Dharma. If I violate these, I will have betrayed the Buddha's teachings, may the vajra of the Buddhas of the three times crush my head. Thus I vow before the Buddhas, please act swiftly according to the vow. Please send down a stream of nectar rain from your mouth, grant it to the yogi me and my retinue, pacify diseases and sufferings. Increase life, power, and prosperity. Increase material wealth, crops and grains, and precious livestock. Curse the lama, slander the dignity of the Three Jewels. This evil deed of so-and-so, who is an enemy obstructing my virtuous deeds. May you use the poisonous wind from your belly, to cause the diseases of the four elements to arise from within. Let the four postures fall to the ground and be struck. Let the five internal organs vomit blood. Let the evil deeds and their retinue be struck in the chest. Let the cries of lamentation resound in the sky. May you kill madness and plague with the poisonous arrow of death. Send down ill omens such as wild beasts, garudas, etc. Swiftly accomplish the deed of protecting the Dharma.
