Old Father Thangtong Gyalpo, while meditating on non-conceptual samadhi at the peak of Glorious Potala Mountain, rainbows and sounds of light appeared in the sky. When I looked into the expanse of the sky, from the land of Orgyen Dakinis, five chief Dakinis came forth. From the mouth of the central chief Dakini: 'In the past, you, brave one, have pacified the suffering of all sentient beings' minds. I rejoice in that. Now, to eliminate all diseases of the body, take these chief medicines.' In the hand of the central chief Dakini: myrobalan (诃子, Arura) balancing the humors, six parts; In the hand of the eastern chief Dakini: stalactite (钟乳石, Congzhi) gathering the essence of existence, half of that; In the hand of the southern chief Dakini: blue medicine (蓝色药, Ngosmen), the all-creating king, equal to that; In the hand of the western chief Dakini: rock extract (岩精, Brakzhun) with the essence of the five precious jewels, half of that; In the hand of the northern chief Dakini: flower essence and potent medicine (良药, Ngenpo'i Men), Sanskrit: mātṛpuṣkā, meaning 'mother flower'. Combine these five types of medicine in proportion. Grind all other medicines finely, and mix with honey (蜂蜜, Drangtsi, Dru) of flower essence. Make pills the size of mustard seeds, and place them in a precious vessel or skull cup. Above it, on a moon disc, visualize the five classes of Dakinis as before, each with a different color, one face, two arms, the right hand holding a precious damaru, the left hand holding a skull cup, dancing in a posture of stretching and bending, holding a khatvanga. On the crown of the central chief Dakini, visualize the yidam Great Compassionate One, remembering him in an instant. First, recite a million manis (oṃ maṇi padme hūṃ), then dissolve the Great Compassionate One into nectar, and visualize it dissolving into the five Dakinis. 无有损害， 对于风、胆、涎等， 四百零四种疾病， 此药无一不能治。 无需一一区分诸病症， 亦可称为调和诸元素， 亦可称为百病一药。 头与足之间的， 所有疾病一次即可。 卫藏上下中三区的， 所有医师皆荐此药。 其他药物皆可埋藏， 唯此一药利益众生。 传授此药诀窍时， 以佳肴盛满容器， 首先上师饮满一口。 讲述此诀窍之历史， 观想上师口中甘露水， 融入弟子之身心。 弟子饮尽剩余之药。 毒药等诸多诀窍， 虽曾宣说然已遗忘， 对此应精进修持。 变味药如下所示： 风病配以沉香、肉豆蔻， 胆病配以苦草、金盏花， 涎病配以豆蔻、桃叶， 热病加狼毒、麝香， 时疫加白附子、诃子。 五脏配以六妙药：心之妙药为肉豆蔻， 命脉之妙药为肉豆蔻， 肺之妙药为莪术， 肝之妙药为藏红花， 脾之妙药为豆蔻， 肾之妙药为小豆蔻。 六腑亦有六妙药：胃之妙药为豆蔻， 胆之妙药为金盏花， 肠之妙药为白头翁， 生殖器之妙药为菖蒲， 膀胱之妙药为海盐， 子宫之妙药为石灰华。 子宫与三世同体。 血清加金腰、水银。 虽无其他对治药， 然根本之药普受赞扬。 莫忘空行母之口诀精华， 我，铁桥疯子， 殷勤记录成文字。 愿以此善行，于一切时处， 所有受我掌控之众生， 皆无疾病痛苦， 并能登上殊胜药师之， 至高果位！ 成就者红丸在此。 顶礼洛给夏瓦拉（Lokéśvara，观世音菩萨）。 老父唐东杰布（Thaṅ-stoṅ-rgyal-po）为利益众生，示现病容而住之时，大悲观世音菩萨（Avalokiteśvara）降临并赐予此诀窍。 白狼毒、狼毒、驱魔古古（Gugul），各一份。 一切之王：党参，虎肉，减半。 太阳之子：朱砂，与上述等量。 一切精华之精华：甘露月之子：诃子，每次服用三份。

The altered medicine is as follows: For wind disease, add agarwood, nutmeg, For bile disease, add gentian, calendula, For phlegm disease, add cardamom, peach leaves, For fever, add aconite, musk, For epidemics, add Typhonium giganteum, chebulic myrobalan. Five organs are matched with six excellent medicines: the excellent medicine for the heart is nutmeg, The excellent medicine for the life force is nutmeg, The excellent medicine for the lungs is Curcuma zedoaria, The excellent medicine for the liver is saffron, The excellent medicine for the spleen is cardamom, The excellent medicine for the kidneys is small cardamom. The six hollow organs also have six excellent medicines: the excellent medicine for the stomach is cardamom, The excellent medicine for the gallbladder is calendula, The excellent medicine for the intestines is Pulsatilla chinensis, The excellent medicine for the genitals is Acorus calamus, The excellent medicine for the bladder is sea salt, The excellent medicine for the womb is travertine. The womb and samsara are considered one. For serous fluid, add catechu, mercury. Although there are no other antidotes, The root medicine is praised by all.
