In the last of the five hundred times, The command of Master Padma will also be broken. Above, the planetary demons, and below, the serpent demons. Gods, demons, eight classes, and mothers are in conflict. Diseases, weapons, famines, and wars arise. Four hundred and forty-four rage like a storm. Sudden human and livestock diseases are abundant. People die impermanently and suddenly. Plague, fever, and joint pain, three, seven, and six arise. Sentient beings are pitiful. At that time, Guru, look with compassion. Thus asked. Master Padmasambhava said: Emaho! Yeshe Tsogyal, the Wisdom Dakini! Do not be distracted, listen to my command. In the dregs of the five hundred times, When the life span is torn to forty, When the Mongol border wars flourish, When the merit of Tibet is exhausted, The Buddha's teachings cause internal strife. Sectarian disputes cause the Sangha to conflict. The supports of body, speech, and mind are destroyed. At that time, various non-virtues arise. Plague, fever, joint pain, and seven sixes. Without leisure, they suddenly die. In order to avert such bad omens, The heart son of Master Padma, Named Dechen Lingpa, The Dharma treasure hidden by Padma and me will emerge. Use the medicinal Dharma treasure in a hidden way. In secret, establish the six realms in happiness. Many fortunate ones enter the Dharma. Relieve the suffering of sentient beings in the six realms. Protect with compassion like the stars in the sky and the sunlight. Thus he said. The chapter of asking and answering is the first. Om Ah Hum! Emaho! Yeshe Tsogyal, the Wisdom Dakini! Entrance Meditation: Refuge is: Hrih (藏文：ཧྲཱིཿ, Sanskrit Devanagari: ह्रीः, Sanskrit Romanization: hrīḥ, Literal Chinese meaning: Hrih)! I take refuge in the Lama who is the embodiment of all Sugatas, in the Three Jewels and the Medicine Buddha, until enlightenment. In order to eliminate all the suffering of body and mind of all sentient beings, and to attain the state of the Medicine Buddha, I practice the Bhagavan Medicine Buddha. Hrih (藏文：ཧྲཱིཿ, Sanskrit Devanagari: ह्रीः, Sanskrit Romanization: hrīḥ, Literal Chinese meaning: Hrih)! The expanse of my own mind, the Dharmadhatu, is beautiful to behold. My own awareness is the immortal Buddha, the Medicine Guru. His body is blue, the light of lapis lazuli. He has one face, two hands, and three eyes, with his hair in a topknot. His head is adorned with a jeweled crown. His right hand holds a golden vajra arura, and his left hand holds an nectar-filled long-life vase. His two feet are in the vajra posture. He is adorned with jewels and silk streamers, and he dwells clearly in the Sambhogakaya. In his heart, from the blue seed syllable Hrih (藏文：ཧྲཱིཿ, Sanskrit Devanagari: ह्रीः, Sanskrit Romanization: hrīḥ, Literal Chinese meaning: Hrih), I visualize the mantra garland turning to the right. Om Guru Bhaisajya Samudgate Svaha (藏文：ཨོཾ་གུ་རུ་བཻ་ཌཱུརྻ་ས་མུད་ག་ཏེ་སྭཱ་ཧཱ༔, Sanskrit Devanagari:, Sanskrit Romanization:, Literal Chinese meaning:)! May the Lama's blue, authentic foundation be firm. The offering goddesses who emanate from myself fill the sky and the earth, offering to the Medicine Buddha. The seven possessions and the five desirable qualities, the seven royal emblems and the eight auspicious symbols, Mount Meru, the four continents, and all the possessions of gods and humans, I take with my mind and offer to the Buddha Medicine Buddha. Whatever kinds of medicines there are, precious mountains and trees and forests, flowers, good fruits, and uncultivated crops, secluded and delightful places, and immeasurable palaces, I transform them with my mind and fill them completely, offering them to the Bhagavan Medicine Buddha and all the Buddhas and their sons. Praise: Hrih (藏文：ཧྲཱིཿ, Sanskrit Devanagari: ह्रीः, Sanskrit Romanization: hrīḥ, Literal Chinese meaning: Hrih)! Bhagavan Medicine Buddha, your body is blue, with the light of lapis lazuli. Even if there is no opportunity to speak directly, please make it appear in my mind what the diseases are and what remedies are effective! By the power of the compassion of the Bhagavan Medicine Buddha's practice, may I close the door to the lower realms, attain the path to the higher realms, and gradually quickly attain unsurpassed enlightenment! In the future, in the world that reaches the end of the sky, wherever the age of disease prevails, may I appear as a physician.
