菩萨以，未来众生无依怙者们，极善观照后加持之。如是说。大自在的黑暗消除者，以秘密真言加持之，世间天众皆赞叹，秘密真言力不能动摇之。如是说。此处说： 诸佛与佛子持明成就众，为利有情咒语无量种，随需所用汇集此善举，愿诸众生受益终成佛。如是说。此乃因见疾病咒语一一亦有多种不同，故不能全部收集。众多咒语，如是修持亦不可行。然精深咒语，任一善持诵，一切目的皆可成就。然新旧诸贤哲之口诀多种多样浩瀚无边，尤其莲师之教言，咒语宝藏一一亦浩瀚无尽。然全部汇集一处亦难得，又因字迹错误等疑惑不定，及来源是否可靠精深无法分辨等，故实修亦难得。以利益他人之清净心，收集最纯正、易修持、精深且来源可靠之咒语，汇集一切所需者，名为麦彭蒋华嘉威多杰。从前开始，凡所能找到之各种咒语，直接抄写的集合，再次于胜生善妙水虎年藏历五月十七日起，按顺序整理成一部著作。于该月二十三日完成，又从原本中添加了许多补充内容，于火羊年藏历三日，作为子本精妙抄写此书。愿一切有情之疾病与痛苦彻底断绝，获得无惧大乐，成就如来药师佛之恒常胜妙果位。愿吉祥！再次，若发现此处未收录之某些疾病之精深咒语，且可信来源可靠，亦应补充于此。具缘者当如是行持，愿善妙增盛。吉祥！所有三个咒语部分的咒语，均已与原始手稿核对，确认无误。

The Bodhisattva, looking upon future sentient beings who are without protectors, blessed them thoroughly. Thus it is said. The great self-existing one, the remover of darkness, was blessed by the secret mantra, praised by the worldly gods, and the power of the secret mantra could not be shaken. Thus it is said. Here it is said: Victorious ones, victorious sons, vidyadharas, and accomplished assemblies, for the benefit of beings, have countless kinds of mantras. By collecting whatever is needed, may all beings benefit and ultimately attain Buddhahood. Thus it is said. This is because even a single disease mantra has many different forms, so it is impossible to collect them all. It is also impossible to practice so many mantras. However, by reciting any profound mantra well, all purposes can be accomplished. However, the instructions of the new and old scholars and accomplished ones are vast and varied, and especially the instructions of Guru Padmasambhava, each treasure of mantras is vast and endless. However, it is difficult to gather all the examples in one place, and it is difficult to practice because of doubts about errors in the script and the inability to distinguish between reliable and profound sources. With the pure intention to benefit others, the one who collects the most pure, easy to practice, profound, and reliable mantras, gathering all that is needed, is called Mipham Jampal Gyepe Dorje. From a long time ago, all the various mantras that could be found were directly copied into a collection, which was again arranged in order from the seventeenth day of the fifth month of the Water Tiger year of Rabjung, and made into one text. It was completed on the twenty-third day of that month, and many additions were made to the original, and this book was finely copied as a child copy on the third day of the Fire Sheep year. May the diseases and sufferings of all sentient beings be completely cut off, and may they attain the fearless great bliss, the eternal and supreme state of the Victorious Medicine Buddha. May it be auspicious! Again, if some profound mantras for certain types of diseases that are not included here are found to be trustworthy and reliable, they should be added to this supplement. Fortunate ones should act accordingly, may virtue and goodness increase. May it be auspicious! All the mantras in the three mantra sections have been checked against the original manuscripts and confirmed to be reliable.
