Add the secret name of "So". Due to reasons such as being young, ignorant, disrespectful, lacking vows, and having many afflictions, transgressions that violate the Vajrayana precepts, such as slandering the guru, which are root downfalls, and branch downfalls such as the eight categories of gross transgressions, whatever occurs, must be confessed and repented before the great Vajradhara Guru and the Vidyadhara Sangha. If I confess and repent, I will abide in a state of touching bliss. If I do not confess and repent, it will not be so. Say this three times. The eldest first asks: Do you see these transgressions as transgressions? The younger ones answer: Yes, we see them. The elder asks again: Will you guard them well in the future? The younger ones answer: We will guard them. After asking and answering in this way three times, the confessor says: This is the means. The offerer says: Good. Say this. Perform three prostrations. Great Vajradhara Guru and Vidyadhara Sangha, please kindly attend to the ritual of rectifying the downfalls of mantra, thank you very much. The confessor answers: I rejoice in your actions in accordance with the Vajrayana precepts. After answering in this way, return to your seat. Then, the elders on the left rise and perform three respectful bows or prostrations. From "Please listen" to "rejoicing" do as just said. This is changing the words of rectifying the downfalls of the Pratimoksha vows. If you want to confess with the actual words of the tantras, then perform prostrations and "Please listen", and ask for forgiveness as before. Prayer: Protector, I am ignorant and deluded, I have violated and broken the vows. Guru Protector, please protect me, Lord Vajradhara! Possessing the nature of great compassion, I seek refuge in the Lord of beings. Body, speech, and mind, root and branch vows, all violations and breakages, I confess and repent. Please bless that all accumulations of sins, obscurations, faults, and stains may be purified. Say this three times, and perform the guarding of views and means, etc., as before.
