Chapter Twenty-two: On the Place of Instrument Therapy Chapter Twenty-two: On the Place of Instrument Therapy Then, the Rishi Yid-las-kye (Born from Mind) again asked: "Respected teacher, Rishi Rigpa Yeshe! How should I learn instrument therapy? Please, the king of physicians, the life-giver, explain." Having asked this, the teacher said: "Great Rishi, listen! For the antidote to cure diseases, learning instrument therapy is of two types: instruments and the implements of instruments. That which extracts diseases from the outside of the body and pacifies them is an instrument. It is also divided into three types: gentle, rough, and forceful. Gentle instruments include three types: fomentation, bathing, and application. Rough instruments include three types: bloodletting, cauterization, and blowing. Forceful instruments include four types: breaking open, cutting off, pulling out, and extracting. Its implements include instruments for examining foreign objects, drying instruments, needles, probes, small knives, etc. Among these, the first is the instrument for examining foreign objects, a round shape without a tip or base, six finger-widths long. Eight finger-widths long, with a blade and a star-shaped hole at the tip. It is used to examine whether a swelling has matured into pus. It has a tubular shape, like a cow's udder, three finger-widths long. Thick, five fingers for males and six for females. For observing diseases, it should have two holes. For operations, it should have one hole, like a lung lobe. Like that, it is used for observing and cutting hemorrhoids. The category of drying instruments for extracting foreign objects is: a lion's mouth and a heron's, a crow's beak. Eighteen finger-widths long, with joints fixed with nails. The root has the shape of an iron hook, with a ring attached. It is used to extract foreign objects stuck in bones. Like that, eight finger-widths long, like a beak of a turquoise grain. Very slender, it is used to extract foreign objects from muscles and tendons. It has a tubular shape, twelve finger-widths long, secret and hard. Gathering from the inside, it is called a reed beak. It is used to extract foreign objects that have sunk deep into wounds. Small teeth, the root bound with a ring, are tsu-mo and de-te, used to extract dead blood vessels and tendons. Showing the category of needles for cutting muscles and releasing blood. Like the tubular shape of a small bird's feather, six finger-widths long.
