From the state of emptiness, upon a lotus and moon seat, The syllable Hūṃ of my own mind radiates light, accomplishing the two purposes. Gathering back, I become the Bhagavan, the Medicine Guru, With a body of blue color, one face, and two arms. The right hand makes the gesture of supreme giving, holding an arura fruit, The left hand in equipoise, holding a nectar-filled begging bowl. Adorned with the splendor of marks and signs, wearing the three Dharma robes, Seated with both legs in the vajra posture. To the right, the prince Shining Like the Sun, red in color, The left hand holding a lotus marked with the sun. To the left, Shining Like the Moon, with a white-colored body, The left hand holding a lotus marked with the moon. Both make the gesture of supreme giving with their right hands. Adorned with silk and precious jewels, to the main deity, With a respectful attitude, standing on a lotus and moon seat. The three places of all the main figures and retinue are marked with the three syllables. From them, light radiates, all the victorious ones of the ten directions, Transforming into the form of the Bhagavan, the Medicine Guru. In the sky before me, Vajra samāja (藏文：བཛྲ་ས་མ་ཛཿ，梵文天城体：वज्रसमाजा，梵文罗马拟音：vajrasamāja，汉语字面意思：) Jaḥ hūṃ vaṃ hoḥ (藏文：ཛཿཧཱུཾ་བཾ་ཧོཿ，梵文天城体：जः हुं वं होः，梵文罗马拟音：jaḥ hūṃ vaṃ hoḥ，汉语字面意思：) dissolve into me, inseparable. At my heart, on a moon, a Hūṃ surrounded by a mantra garland, Light radiates, offering to the noble ones, purifying the two obscurations of all beings. The compassion and blessings of the king with his sons, Gather back, blessing my being. Just hearing his name eliminates the suffering of the lower realms, The Buddha, the Medicine Guru, who cures the three poisons and diseases, I prostrate to the light of lapis lazuli. Bhagavan, Tathagata, Arhat, Perfectly Enlightened One.

祈请圆满正等觉药师佛， 赐予我与无边众生， 殊胜与共同之成就。 如是赞颂祈请后，以 嗡 班匝 穆（藏文：ༀ་བཛྲ་མུཿ，梵文天城体：，梵文罗马拟音：oṃ vajra muḥ，汉语字面意思：嗡，金刚，穆）祈请返回本位。眷属与座垫化为光融入自身。观想自身三处为嗡 阿 吽（藏文：ༀ་ཨཱཿཧཱུྃ，梵文天城体：，梵文罗马拟音：oṃ āḥ hūṃ，汉语字面意思：嗡，阿，吽）所标识。以此善根愿我等，速疾成就药师佛，等回向发愿。若修近分，则安住于沐浴与洁净中，念诵短咒百万遍，并加念补阙。座间可作广略随宜之顶礼供养，或读诵药师经大小本，随力而为。此乃为应所需，由多闻之瑜伽士妙音（mān̄jughōṣa）所造。愿此善根成为利益一切有情之安乐之因。吉祥！ 药师佛之极简修诵法：利益安乐之速道。妙音。

To the perfectly complete Buddha, the Medicine Guru, I beseech you to grant me and all limitless sentient beings, Supreme and common attainments. Having praised and prayed in this way, request their departure with Oṃ Vajra Muḥ (藏文：ༀ་བཛྲ་མུཿ，梵文天城体：，梵文罗马拟音：oṃ vajra muḥ，汉语字面意思：Om, Vajra, Muh). May the retinue and seat dissolve into light and merge into oneself. Visualize one's own three places as marked by Oṃ Āḥ Hūṃ (藏文：ༀ་ཨཱཿཧཱུྃ，梵文天城体：，梵文罗马拟音：oṃ āḥ hūṃ，汉语字面意思：Om, Ah, Hum). Through this virtue, may I quickly, Attain Buddhahood as the Medicine Guru. Etc., dedicate and make aspirations. If doing the approach, then abiding in bathing and cleanliness, recite the short mantra one million times, along with the supplementary recitation. In between sessions, perform prostrations and offerings, elaborate or concise as appropriate, or read the Medicine Sutra, large or small, as much as one is able. This was composed by the much-learned renunciate Mañjughoṣa for the sake of necessity. May this virtue become the cause of benefit and happiness for all beings. Mangalam!
