顶礼文殊菩萨！ 皈敬三宝！ 皈敬圣观自在菩萨！ 菩萨萨埵！ 摩诃萨埵！ 大悲者！ 即说咒曰： 嗡（oṃ），阿嘎喋， 毕嘎喋， 尼嘎喋， 嘎当嘎喋， 嘎若喋， 嘎若达， 毕热耶， 梭哈（svāhā）。 于彼薄伽梵前，以青稞粉不落地，分别作八个坛城，每个坛城念诵十三遍，剩余青稞粉念诵咒语，涂于病人身上，一切疾病皆得平息。若七日、十三日或二十一日成就，则非犯五无间罪。若未能如是成就，则我犯五无间罪。狮子吼所立誓之陀罗尼圆满。印度堪布语自在及译师克洛嘉·喜饶孜所译。

Homage to Manjushri! Homage to the Three Jewels! Homage to Arya Avalokiteshvara! Bodhisattva! Mahasattva! Great Compassionate One! Namely, the mantra: Om（oṃ）, Agaṭe, Bigaṭe, Nigaṭe, Kaṭaṃ Kaṭe, Karoṭe, Karota, Birye Svāhā. In front of that Bhagavan, without letting the barley flour fall to the ground, make eight separate mandalas. Recite thirteen times for each mandala, and recite the mantra over the remaining barley flour. If applied to the sick person, all diseases will be pacified. If accomplished in seven, thirteen, or twenty-one days, then it is not committing the five heinous crimes. If it is not accomplished in this way, then I will commit the five heinous crimes.
